A child taking part in a performance, which can include TV or filming, theatre, sporting activities or modelling, must be chaperoned, either by their parent or a registered chaperone. Registered chaperones are licensed by the local authority in which they live. To apply for a chaperone licence you will need to download, complete and return the ‘Application to act as a Chaperone’ form.
Please attach two recent passport size photographs with your application. One of these will be attached to your licence as a means of identifying the holder of the licence; the other will be kept on record to be used by authorised officers of the authority when making visits to performance venues.
References will be sought from the people given on your application form; in addition the CRB Disclosure will check records relating to access to children. The Criminal Records Bureau will send the disclosure directly to you.
When we receive your application, we will send you a CRB Disclosure form. Once you have completed this form please contact us on 01753 787670 for an appointment to come in to this office (which is located at The Town Hall, Bath Road, Slough).
Please note that this process can take time, particularly at peak times of the year such as before Christmas and it is necessary to apply for your licence in good time. Whilst every effort is made to process all applications as quickly as possible, please ensure that your completed forms arrive at least six weeks before the licence is required. It is not possible for unlicensed people to act as chaperones.
Your initial licence will be valid for 12 months and may then be renewed for a further period of 2 years. To renew, please return your licence to this office, at least one month prior to expiry, for updating. After this period it will be necessary to reapply for a new licence. Please request the application pack at least six weeks prior to the date your licence expires.
