If you have answered yes and this is making your daily life difficult then the Better Care Higher Standards Charter is for you!
The charter can be found below. It's produced for people over 18, living in Slough, who have a long term illness or disability, or who care for someone who does.
The charter gives you information about the standards of service you should expect to receive from housing, health and social services. It also contains useful phone numbers of organisations who might be able to help you.
