Persons severely mentally impaired discount/exemption

If you or your partner is considered to be severely or mentally impaired, you may be entitled to a discount or an exemption from your council tax.

Please see the guidance notes on the form for clarification on whether you may meet the criteria needed.

On reviewing the guidance notes if you believe that you are entitled to this reduction please download the 'Council Tax: Application for Discount/Exemption for the Severely Mentally Impaired' form and complete and return it to Council Tax Section PO Box 1733, Slough, SL1 1FS. Alternatively you can hand it in to the Customer Service Centre.

Please note Section 3 of the Form must be completed by a doctor.

Failure to provide the full information requested may delay the application being assessed.
