What might my care provider need to know about me?
The Alzheimer’s Society have produced a useful leaflet ‘This is Me’ which covers this well. This includes:
- the name you wish to be known by
- the person who knows you best
- your hobbies and interests
- things which upset you
- how you relax
- hearing and eyesight
- mobility
- allergies
- medication
- faith needs.
