Updating your name or address
If you have changed either your name or address you will need to inform the Licensing team of your updated details.
Once the form has been received and, we will then update your licence.
When you complete the form, you will need to provide copies of certain documents as evidence.
Change of name
If you are changing your name, you will need to provide copies of the following documents showing your updated details:
- passport
- DVLA driving licence
- one of the following:
- change of name deed
- marriage certificate
- divorce document.
Change of residential address
If you are updating your address, you will need to provide a copy your updated DVLA driving licence (both sides) with your new address. If you have do not have your updated DVLA licence, you can email licensing@slough.gov.uk a copy when it arrives.
