What is the reablement and independence service

This service offers up to six weeks intensive support and therapy in your own home, care home or rehabilitation bed. This is to help you:

  • rebuild strength
  • increase mobility
  • regain confidence after an illness or injury
  • regain all or most of your independence.

The service is available 365 days a year to support:

  • hospital discharge
  • community rehabilitation
  • a response to community crisis.

Reablement is free of charge until you leave the service.

After six weeks a Reablement worker will assess and arrange long term care if required. Long term care is means tested and a contribution maybe required.

Eligibility criteria

This service is free and available to all Slough adults if:

  • you are 18 years or over
  • the initial assessment suggests you could benefit from this service
  • you are registered with or willing to register with a GP
  • you are medically stable and do not need acute medical care (unless on a virtual ward)
  • you are willing and capable to engage with Reablement staff as far as reasonably practical
  • your home is safe for the service to work in.
